Ability to lock the birthdate & name enroll form fields

Lock birthdate and name fields so only admin users can edit them

Emergency contact not allowed to be same as either account holders

If there's a possibility to prohibit the use of the same name/phone number as the account holders (both primary and secondary parent) so we can have a different contact to reach for emergencies. Thanks.
